What do I do if my patient did not receive their intake form?

Although most intake forms which are emailed to the patients from SOAP Vault do get successfully delivered, it is possible that some may not.  Sometimes they can end up in the patient's spam folder, or even be filtered out before it even gets to the patient.  

 

  1. Go the Dashboard
  2. In the Incomplete Intake Form pane click on the Black Triangle that right of the patient's name. 
  3. You have three options.  One, you can Edit the patient details. Two, you can Resend Intake Form. Three, you can Get Link and  copy and paste the link and send the it from your email.

    Do you see a difference between users that are on apple vs pc for having problems with receiving or accessing the link for initial intake? I have one person where her link sends her only to the main page of soapvault.com and not the intake form....

    Hi Heather, it's possible that some email clients will not display the links as intended. We have a correction for this coming in a very near future update. The issue is not specifically Apple vs. PC though.

    Can a blank version of the intake form be printed, for a patient to complete on paper.

    Hi Walter, yes you can. Just click "+ add" then "patient". On the "Add Patient" page there is a button to print a blank intake form.
