What do I do if my patient did not receive their intake form?

Although most intake forms which are emailed to the patients from SOAP Vault do get successfully delivered, it is possible that some may not.  Sometimes they can end up in the patient's spam folder, or even be filtered out before it even gets to the patient.  

In situations where the patient does not recieve their intake form, we suggest for you to send the link directly to the patient.  This can be done on the Dashboard by clicking "Pending Intake Forms", in this dialog box you can click "Get URL" and copy/paste the URL into an email and send directly to the patient from your own email account.

We are always trying to improve our email delivery rates and will continue to do so.

    Do you see a difference between users that are on apple vs pc for having problems with receiving or accessing the link for initial intake? I have one person where her link sends her only to the main page of soapvault.com and not the intake form....

    Hi Heather, it's possible that some email clients will not display the links as intended. We have a correction for this coming in a very near future update. The issue is not specifically Apple vs. PC though.

    Can a blank version of the intake form be printed, for a patient to complete on paper.

    Hi Walter, yes you can. Just click "+ add" then "patient". On the "Add Patient" page there is a button to print a blank intake form.
