Collecting Patient Signatures on Forms

You can collect patient signatures on Forms in a variety of ways.

 

Email Forms for patients to review and sign prior to appointment, or while in your waiting room.

  1. Once you create a form for a patient, the form will be listed within their profile.
  2. Click the Dropdown arrow for the desired form.
  3. Click Email Form.
  4. Modify the default message and click Send.
  5. One the patient signs the form, the form will be locked and timestamped.
  6. (optionally) You could have clicked Get Link and manually emailed the link to the patient to sign.

In Person Signatures

  1. Once you create a form for a patient, the form will be listed within their profile.
  2. Click the Open Agreement.  This will open a new tab for the patient review.
  3. Allow the patient to review and sign the form.
  4. Once signed, close the web browser tab which had the agreement.  (You may need to refresh your page at this point to see the updated status of the Form).

    If we have a touch screen computer, can the patient sign forms directly in the soap note on the computer or sign the form and have it attached to the soap note?

    Yes, they can sign on the screen.  Forms are automatically linked to SOAP notes when the signature date and treatment date match.  You can see an example in this video here: https://youtu.be/XrN7kjVDQ0M?t=2m50s
